Care Coordinator

A council in the South East is using us to help them recruit a Senior Occupational Therapist for a Pathway 3 project. They have received a chunk of better funding and are using it to run a pathway 3 project which is focussed on hospital discharges into nursing homes and new residential settings. You'll be the only Senior Occupational Therapist on the project and will be working with 5-10 patients per week. You&nbsp;will be conducting assessments and creating care act compliant reports, prescribing care packages and&nbsp;working with multi-agencies (hospital, care &amp; residential homes) to ensure successful transitions from the hospital bed to the home. You will need to have experience with complex moving &amp; handling, and a strong understanding of the care act. The council has funding for up to a year, they are going to start with a 3 month contract initially but there is a great chance that this role extends for the full year. The council understands the complexity of this role and is paying a competitive hourly rate to ensure they find the right person.
