Functional Assessor - Nurse, Paramedic, Physiotherapist or Occupational Therapist

We now have an opening with our client for an Occupational Therapist to join on an initial 3-month contract. There is a possibility this will be extended further. Your main purpose in this role is to undertake Occupational Therapy assessments and interventions within the London area. Principal responsibilities: * Work as part of the Reablement service ensuring compliance with CQC requirements. * Manage hospital discharges process, assessing risk and activities of daily living * Assess and advice services users on all aspects of activities of daily living seeking at all times to maximise independence and seek creative home based alternatives to admissions to care home or hospital * To be able to record using MOSAIC * To complete full OT assessments within national and local targets. * Manage and provide community paediatric occupational therapy, including assessments and technical support.
