Start date: ASAP AKTON Recruitment is working closely with the local authorities and we are searching for a Occupational Therapist in the Haringey area. 3 - 6 months contract with the local authorities Enhanced DBS required. Job Purpose:
1. To provide an initial assessment of service users with complex care and therapy needs across the teams within the Access and Independence Service: First Response, Reablement and the Assessment Service.
2. Support service users to set client centred goals to promote independence and wellbeing in the home environment. Facilitate service user choice and empowerment through promoting independence, the timely provision of information, advice and support using evidence-based interventions to ensure service users maintain independence for as long as possible.
3. Assessment and provision of equipment to improve independence with activities of daily living and to reduce risk of injury. This includes manual handling assessments to reduce risk of injury to service users and carers and, where feasible, reduce packages of care for single handed care.
4. To independently manage a caseload, utilising effective prioritisation, delegation and time management skills to enable effective throughput of service users.
5. To identify service users appropriate for Continuing Health Care to manage complex health needs
6. To identify Safeguarding concerns, complete thorough risk assessment and refer where appropriate to services to manage risk of harm to service users. This includes situations where people may be neglected or abused in accordance with the Inter-Agency Policy and Procedures for Safeguarding Adults
7. To assess capacity and ability to make informed decisions relating to assessment process as per Care and Mental Capacity Act.
8. To ensure cost effective service provision and make recommendations for specialist equipment and adaptations in accordance with eligibility criteria and departmental guidelines.
9. To complete carers assessments to support informal carers in their role and manage risk of carer breakdown
10. To keep accurate and up to date case records and written reports using council's IT systems and in line with statutory and local protocols. Ensure the highest level of data quality for all information recorded, ensuring that the information is consistently timely, accurate and complete to ensure the service meets the Professions and statutory recording requirements, including case recording on Mosaic. Knowledge, Qualifications, Skills and Experience Diploma of Occupational Therapy/B.sc. Occupational Therapy.
HCPC Registration
Undergraduate or post qualification experience in completing therapy assessments and interventions in a variety of clinical settings
To show an ability to manage risk and identify potential risk to service users
Ability to present information and undertake written reports

You will be based at our Gravesham Community Hospital location on our Sapphire Ward which comprises of 15 beds for Neurological conditions and 6 beds for general rehabilitation. As a Band 6 Occupational Therapist you will provide comprehensive assessment, advice and treatment of patients with diverse Neurological presentations and complex physical, psychological and cognitive conditions. You will be responsible for managing a clinical caseload of patients whilst working alongside our talented multi-disciplinary team to provide patients with the identified support and equipment to promote independent living. The Ideal Candidate Our idea candidate will: * Have a degree or Diploma in Occupatoinal Therapy * Have current and active registration with HCPC * Membership of the British Association of Occupational Therapy * 18 months post graduate experience * Experience of working in the field of neurological rehabilitation * Knowledge and understanding of the NICE guidelines and relevant NSF’s * Involvement in clinical audit * Ability to travel independently between sites as required by the service As this role may involve travel between services a UK Driving License and access to a car are a requirement for this role
