Job overview

The Prehabilitation service at CHFT aims to improve the outcomes of newly diagnosed cancer patients. The role will facilitate the development and delivery of a model of support using or adapting existing services and delivering the Active Together model to support the psychological wellbeing, dietary and physical activity needs of newly diagnosed cancer patients.

Working in collaboration with partner organisations and Yorkshire Cancer Research the postholder will implement the Active Together prehab and rehab model.

The Active Together programme aims to maximise resilience to treatment, improve longer term health, minimise side effects and empower patients to be partners in their healthcare. The postholder will facilitate the development and implementation of Active Together, a 5-year funded programme with staged implementation into all relevant cancer pathways.

The postholder will be a dynamic, motivated individual who is a self-starter, able to adapt to new strategies, ideas, and ways of working and they will strategically lead a team of staff and be expected to inspire the team to carry out a new and potentially challenging work programme. Excellent project management and organisational skills are required to ensure the project delivers outcomes both in place and across the locality. The role requires excellent communication and negotiation skills to influence stakeholders across a range of teams, organisations and localities.

Main duties of the job

Overall responsibility for the successful initiation, planning, design, execution, monitoring, controlling and expansion of the service.

Management of core, senior members of staff and participate in

teaching and support of other team members.

Manage audit and quality improvement data collection.

Accountability and responsibility for multiple budgets and income

streams.

Contribute to the highest standards of clinical governance and

professional standards.

Deliver elements of Prehab in line with own professional skills.

Work closely with partners identifying appropriate interventions to

reduce health inequalities and address the wider determinants of health.

Work with partners to plan, monitor and report on progress

to peers and senior colleagues, proactively escalating risks where

appropriate and implementing solutions.

Be the visible and accessible point of contact for advice, guidance, and support on all aspects of the service.

Develop and engage with appropriate patient groups to ensure proposed solutions are in line with what is required of those diagnosed with cancer.

Responsible for carrying out research and development activities to ensure best practice and innovation is embedded across all aspects of early diagnosis programmes.

Working for our organisation

We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients' homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.

We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children's and young people's services; end of life care and outpatient and diagnostic imaging services.

We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.

Detailed job description and main responsibilities

Planning and Organisational Skills
• Provide excellent project management and co-ordination of a broad range of complex activities within this significant service across a range of partners and stakeholders. Many of which will require constant re-assessment of delivery timescales and plans.
• Develop and maintain a mixed portfolio of core project documentation, working to agreed standards in project management methodology commensurate with the scope and complexity of the initiatives being supported.
• Liaise closely with all health and social care providers and create strong networks which promote a collaborative approach to service improvements across the integrated care system.
• Ensure strong communication at all levels regarding service changes
• Work with all teams to inform and influence evidence-based practice within service redesign and improvements.
• Proactively manage stakeholders, respond to and resolve conflict between different stakeholders when this arises through facilitation or other appropriate mechanisms.
• Monitor, record and report progress made within each project/ activity within the work stream ensuring reporting and appropriate escalation of key risks and issues which may impact on project success through the relevant project governance.
• Maintain up to date awareness of new developments and resources available for the assessment and treatment of patients within the specialist area.
• Contribute to the preparation of submissions to secure, support and funding for proposed innovations.
• Liaise with directorates, stakeholders and other professions regarding service planning and development as required.
• Set up and maintain steering / project group meetings, for relevant areas of development, at the required frequency depending on project stage, ensuring accurate taking of formal notes to inform revised project plans and approach in the development of innovation and service development.
• Manage own workload and make informed decisions independently on behalf of steering / project groups with appropriate and timely escalation of relevant risks and issues to the Cancer Management Team or other designated contact.
• Actively be aware of and involved with ongoing research and to promote research activities within the wider team.

Analytical and Problem-Solving Skills
• Participate in the statistical analysis and presentation of information to inform and make decisions on aspects of the service including pathway re-design and service improvement plans. This will be complex as the data will often be gathered from multiple sources and clinical systems and the post holder will be required to present a range of options which will inform and support decision making and re-design of pathways.
• Present complex information to groups of stakeholders, where appropriate including analysis of primary and secondary care data and statistics used to inform project planning and also presentations at events relating to project progress and delivery models.
• Co-ordinate, together with Sheffield Hallam University, the implementation of evaluation plans for the service, ensuring effective systems in place to gather data and feedback at key points from a broad range of stakeholders. This will evidence the success of local projects and new service developments including the use of statistical mechanisms such as surveys and audits.

Professional and Managerial Duties
• Ensure effective communication, influencing and negotiation, acting as a key link between key stakeholders to promote the delivery of a feasible and sustainable prehabilitation and rehabilitation service. This will often involve careful negotiation where delivery plans need to be changed whilst managing stakeholder expectations.
• Use a range of influencing and negotiation skills and strategies to ensure delivery from potential contributors for whom there is no direct line management responsibility.
• Employ negotiation skills and expert specialist knowledge in the management of conflict across a range of situations as it arises e.g., resolving differences in opinion.
• Deliver elements of the prehabilitation and rehabilitation model to patients within the scope of professional practice.
• Work as an autonomous practitoner, clinically, legally and professionally responsible for managing own caseload with the ability to prioritise, organise, allocate, delegate and triage referrals to optimise patient care.
• Provide a leadership style which is underpinned by values around equality, diversity, and openness, efficiently building and maintaining relationships with the cancer MDTs and other key individuals across the organisation.
• Ensure that individual objectives are clearly defined within the wider workforce framework and in-line with CHFT objectives, using the appraisal process as a vehicle for this.
• Always ensure cover for the team, engaging temporary support staff as required in accordance with CHFT policies and procedures ensuring induction, supervision, and delegation of workload.
• Contribute to local and national prehabilitation and rehabilitation, cancer, and relevant network meetings.
• Attend meetings with a range of partners at key points within the service development to report progress. This may require formal and informal presentation of projects / innovations and workstream progress and relevant information to different audiences.
• Work closely with primary and secondary care groups across Calderdale and Huddersfield to ensure excellent levels of engagement and buy in to the development of service changes and transformation models as they are developed.
• Work with patient groups and patient leaders to ensure appropriate levels of patient engagement is embedded into the service and projects within as they are set-up and implemented, ensuring involvement in key aspects of service development work and ensuring support is available to patient leaders when and if required. The post holder will then ensure that the findings and feedback from patients are used to inform any re-design aspects of the services.
• Support a learning culture across organisations through the effective dissemination of learning outcomes from the service.
• Take part in the support, development and leadership of staff recruited into services and roles that support the delivery of prehabilitation and rehabilitation and the optimisation of cancer patients' health and wellbeing.
• Effectively lead staff to deliver results. Inspire others to be positive in their continuous professional development.

Financial and Human Resource Management
  • To contribute to the overall financial reporting requirements as required by all key stakeholders.

• Manage individual staff working within the project within the post holders' level of responsibility, this will require excellent resource management skills as you will be working and applying line management techniques with multiple colleagues across a range of different organisations to ensure successful completion of tasks.
• To implement systems to monitor the quality of clinical practice through audit and measurement of clinical risk, complaints, benchmarking and ensure changes to practice are introduced and monitored where appropriate.
• To advise and participate in the investigation of critical incidents and complaints. To advise on/take forward appropriate remedial action and ensure lessons are learnt and shared.
• Support the training and development of staff. Work within relevant HR Policies and Procedures.
• Support the development of job descriptions and the recruitment process of the workforce to deliver the prehabilitation, rehabilitation / Active Together service.

Managing Self
• Participate in regular supervision.
• Complete all mandatory training.
• Participate annually identifying, developing and agreeing your own development plan with your Line Manager using the Trust Appraisal.
• Comply with all Trust policies, procedures and protocols.

Person specification

QUALIFICATIONS / TRAINING

Essential criteria
  • Professional Registration (HCPC/NMC)
  • Masters level qualification or education in a management discipline, (or working towards Masters degree), or relevant experience in a related subject or field, project management, business management, systems and processes / information management
  • Evidence of Leadership Training

Desirable criteria
  • PRINCE2 Practitioner or equivalent professional project management qualification
