Through our talented employed team of over nearly 400 employed First Contact Practitioners & Advanced Practitioners, who are supported by experienced and Health Education England-trained FCP clinical supervisors, we have been commissioned by c 200 NHS Primary Care Networks (PCNs) to deliver an integrated and local FCP managed service. We now serve 1000+ GP practices and a patient population of over 9 million. We have now diversified to include Occupational Therapy in our service offer, and are searching for a dynamic, enthusiastic and driven OT with specialist frailty experience to join our team in Gloucester who is keen to develop their career and support PCP’s OT service line growth. Role Profile This post requires an occupational therapist who is specialised and can work at a high level of practice in primary care. They will use in-depth, expert knowledge, clinical skills, and experience in the management of patient’s, groups, and communities’ occupational participation. The post holder will provide personalised, biopsychosocial occupational therapy interventions from initial clinical assessment, diagnosis, intervention to evaluation. They will provide complex clinical decision-making, and expert care for patients in primary care. They will have a comprehensive understanding of how to use risk assessment for undifferentiated diagnoses and clinical red flags, including when alternative input is required. They will work collaboratively with the primary care multi-disciplinary team (MDT) across pathways and systems. They will meet the needs of patients and carers, offer digital delivery, and support the delivery of appropriate policy and procedures. They will provide occupational therapy leadership across clinical practice, education, and research. The focus of this service will be rehabilitation for older adults to stay well at home. There will be the requirement to use risk stratification tools, rapid access, assessment of the home and short-term interventions including advice on how people can be safely managed. Will also include working into care homes and palliative care, dealing with undifferentiated diagnosis and red flags Our service goals: 1. Admission avoidance 2. Speedy discharge from hospital 3. Enabling the person to remain in their chosen home environment Key Responsibilities Clinical Practice Assess, plan, implement, and evaluate treatment plans, with an aim to to ensure people can cope at home, including support for carers. Use of frailty indexes to identify people needing a proactive approach. Rapid crisis response to prevent hospital admission or to speed discharge. Short term rehabilitation interventions and referral onto specialist services if indicated. Work with patients through a shared-decision making approach to plan realistic, outcomes-focused goals. Undertake both verbal and non-verbal communication methods to address the needs of patients that have communication difficulties. Work in partnership with multi-disciplinary team colleagues, physiotherapists and social workers, alongside the patients' families and carers in treatment planning to aid rehabilitation Leadership Provide leadership and support on occupational therapy clinical service development across your PCN, alongside learning opportunities for the whole multi-disciplinary team within primary care. Lead the development of crisis and contingency plans with relevant professionals to prevent admission to or speed discharge from hospital. This should focus on maximising and maintaining independence in preferred home, work, and community settings. Lead on the development, implementation, and evaluation of a seamless occupational therapy service across primary care. This will include working with health, social care and voluntary sectors, improving standards of patient care and cross boundary working. Lead and manage service delivery by supporting Primary Care Physio in evidencing what best practice looks like and extending our service reach across England. Liaise with the Royal College of Occupational Therapists to support implementation of best practice, and act as a thought leader for OTs working in primary care. Experience, knowledge and qualifications BSc in or pre-reg MSc in Occupational Therapy under a training programme approved by the Royal College of Occupational Therapists (RCOT). Fully HCPC and RCOT registered practicing Occupational Therapist. An up-to-date CPD portfolio demonstrating that you able to operate at an advanced level of practice. Fully verified HEE Roadmap to Practice (Stage 1). Completion of an appropriate level 7 MSc in Advanced Practice (desirable). Job Types: Full-time, Permanent Pay: £45,996.00-£54,619.00 per year
