Pay Rates: £22.99 per hour
Shift Patterns: 37.5 hours, 5 days per week

Herefordshire and Worcestershire Health and Care NHS Foundation Trust are the lead provider of mental health and learning disability services across Herefordshire and Worcestershire supporting children, adults and older people. They work alongside community partners to support people’s mental wellbeing. They are the main provider of Integrated Community Services in Worcestershire, working alongside GPs, social care teams and the voluntary sector to deliver nursing and therapy services which keep people at home for as long as possible.

In this role:
- You will Promote and provide expertise and guidance to the wider multidisciplinary team on a range of social work
- You will Assess and monitor the mental health and social care needs of service users and carers as part of an integrated team
- You will Carry out appropriate review or re-assessment of individuals and/or carers as required
- You will aim to promote the independence and well-being of adults with mental health needs, assist social inclusion and valued community presence and enable the widest possible expression of chosen lifestyle
What you’ll need and be able to provide evidence for:
- You must possess a relevant professional qualification
- You must possess a professional registration with the Health and Care Professions Council
- Demonstratable experience in a Mental Health Profession
In addition to experience, all Qualified roles will be required to provide evidence of
- Relevant Qualifications
- Registration with the relevant Registration Body if applicable
