Duties and Responsibilities:

<li>Conduct medication reviews and optimise prescribing practices within GP practices.</li>
<li>Work collaboratively with GPs and other healthcare professionals to improve patient care.</li>
<li>Provide expert clinical advice on medication use and safety.</li>
<li>Support chronic disease management and deliver patient education to enhance health outcomes.</li>
</ul>
Qualifications:
<ul>
<li>Registered with the General Pharmaceutical Council (GPhC).</li>
<li>Independent Prescriber qualification is highly desirable.</li>
<li>Strong knowledge of clinical guidelines and prescribing best practices.</li>
<li>Excellent communication and organisational skills.</li>
