Duties and Responsibilities:

<li>Conduct medication reviews and provide clinical advice to healthcare professionals.</li>
<li>Collaborate with the multidisciplinary team to ensure safe and effective prescribing practices.</li>
<li>Support patients in managing chronic diseases through medication optimisation.</li>
<li>Deliver training and mentorship to other healthcare staff.</li>
</ul>
Qualifications:
<ul>
<li>MPharm degree and registration with the General Pharmaceutical Council (GPhC).</li>
<li>Experience in clinical pharmacy or hospital settings preferred.</li>
<li>Strong communication and organisational skills.</li>
