Detailed job description and main responsibilities
  • Develop detailed and complex technical understanding of discharge to assess pathways and hub processes to lead service delivery from a point of professional expertise and competence.
  • Work with the Integrated Intermediate Care Coordination Service (ICCS) Manager in assuring safe and effective service delivery including responsibility for data reporting.
  • To deputise for the Integrated Intermediate Care Coordination Service (ICCS) Manager where necessary including attendance at required system or NWL forums.
  • Demonstrate patient centred values ensuring that all patients are treated with compassion and dignity.

Person specification

Qualifications

Essential criteria
  • First level degree or equivalent
  • Evidence of further relevant courses
  • Equivalent significant experience of managing discharge and patient flow
  • HCPC Registered Therapist / First Level Registered Nurse or Therapist currently registered with relevant professional body

Desirable criteria
  • Management training and qualification
  • Mentorship course
  • Leadership courses/qualifications

Exper

Essential criteria
  • Has the required level of knowledge of area of specialty and be able to apply that knowledge in order to promote the health of the patients.
  • Extensive experience at minimum of Band 7 (Agenda for Change) or equivalent - minimum 3years
  • Extensive experience of senior management
  • Experience in team management including performance management
  • Experience of receiving and delivering supervision
  • Able to teach in formal and informal settings
  • Budget Management
  • Proven ability to prioritise and manage own workload with minimal supervision
  • Experience of initiating and implementing changes to services
  • Experience of working collaboratively with a variety of agencies and professionals and across organisational boundaries.
  • Evidence of innovation in practice evaluation and audit skills.

Desirable criteria
  • Demonstrates high level skills to communicate effectively and appropriately with partners, patients, staff and stakeholders
  • IT literate in the use of Microsoft Word, Email and a basic understanding of Excel spreadsheets, data recording tools, e.g. Emis (Patient Activity data and Datix)
