We are working with an NHS organisation based in the South of England, who are looking for a RMN, OT or QSW to work with the Adult Mental Health Team, providing a high-quality service across all areas of the Adult Mental Health Service.
The role will be working at a NHS Band 6 and the AMHTs operate a 24-hour, 7-day week service. Post holders will be expected to provide a flexible service in accordance to the needs of the service and service users.
Job Summary:
The post holder will work within the Adult Mental Health Team, providing mental health treatment in the community for patients with mental needs identified within Cluster 4-17 Supporting patients to work towards their own recovery.
The post holder will act as CPA Care Co-ordinator for a case load of patients. The post holder will also provide home treatment for patients experiencing a mental health crisis.
Key responsibilities:
* Work independently as part of a multidisciplinary team to diagnose, assess, plan and implement patient specific programmes of care/interventions, contributing to the continuous assessment of patients;
* Undertake more complex assessments, using your profession specific clinical knowledge and skills;
* Function effectively across a variety of environments (e.g. ward, department, patients’ homes etc) involving changing and demanding conditions;
