Daytime Healthcare are working with an established Home Care Provider, who are expanding and looking for an Care Coordinator to join their friendly team! You could have the opportunity to work alongside a National Care provider!

Duties Include:

* The successful candidate will coordinate the delivery of a safe and well-planned domiciliary care service that is service user focused

* Responsible for carrying out initial assessments, doing reviews and spot checks

* Creating service user care plans, peform assessments and reviews

* Planning staff rota's using the bespoke system

* Particiape in the branch on call rota

* Supporting the new and existing care staff team and coordinating the key carers in your designated areas

* Carry out staff supervision and appraisals

* Ensuring that all operations are Company and CQC Compliant

Requirements:

* Good organisational and communication skills to ensure an excellent service

* A minimum of 1 year Care Coordinator experience in domiciallry care

* Experience doing rotas and care plans

* Full drivers licence and access to vehicle is essential

* Good IT skills

* Ability to work within a fast-paced enviroment to deadlines

Schedule:

* Full time, 40 hours per week, 9am - 5pm

* On call on a rota basis
