*Aim of post:* A Care Co-ordinator who will be accountable by example, within a team setting, to promote the good name of Delta care Ltd, providing and achieving the highest standard of quality care. To share responsibility for monitoring and supervising staff and service users, preparing rota, providing a smooth and efficient service. Promote Dignity in Care, Safeguard Adults and Children and promote good infection control standards.

*Objectives: *To undertake duties, which are considered, appropriate to the post. Exercise leadership and promote a good relationship between staff, service users and outside agencies.

To be flexible.

Assess the needs of service users and identify problems, plan solutions and effective individual care plans.

*Duties: Be aware* of Inspection procedures undertaken by Care Quality Commission and the Council Contracts department.

Contribute to inspections as and when required.

Work with Management to promote the company and increase business.

Act within Delta Care Code of practice and in a professional manner always.

Co-ordinate care staff rotas

Listen to and Support Staff.

Provide Care to Services Users when and if required.

Liaise with service users.

Undertake weekly telephone reviews for new and existing service users.

Audits when needed.

Staff supervisions and spot checks and reviews when needed

Monitor standards of care

Maintain accurate records

Write reports and Letters when needed

Attend and contribute to staff meetings

Keep accurate records and log information on the care package system.

Move between offices if requested by management.

*Health and safety:* To work within Delta Care Ltd Health and Safety policy.

Ensure the principles and practices of Safeguarding Adults and children and Infection Control are adhered to throughout the work force.

*Hours of duty:* Monday to Friday in office. On call to be discussed at interview but it will follow a monthly rota for on call duties.

*Office hours: Monday to Friday 9am – 4pm*
