Our client delivers nurse-led services in the clients' own homes and works in partnerships with various healthcare organisations to improve patient outcomes and experiences. Their focus is on leading with quality and putting their clients at the heart of everything they do.

As a Care Coordinator, your responsibilities will include:

Coordinating and overseeing the mobilisation of new complex care packages Managing the process of new package set-up including care plans, risk assessments, and compliance with regulatory requirements Developing relationships across the business to ensure the bespoke service runs seamlessly Liaising with key stakeholders including clients, families, healthcare professionals, and internal teams to ensure a smooth and timely delivery of care Package and Benefits: As a Care Coordinator, you will receive: An annual salary of up to £30,000 24 days holiday plus your birthday off A pension scheme A bonus scheme The ideal Care Coordinator candidate will have: Previous experience in care package coordination Proven leadership skills Knowledge of the legislation affecting health and social care Excellent communication skills, both verbally and in writing The ability to gain the trust, commitment and cooperation of others Strong organisational skills A flexible and creative problem-solving ability Numeracy skills and the ability to analyse complex issues, absorb information and understand data If you have experience or interest in roles such as Care Manager, Health Care Coordinator, Patient Care Coordinator, Service Coordinator, or Health and Social Care Coordinator, this Care Coordinator role could be the perfect fit for you.
