Care Coordinator - £30,000 - £34,000 based in Wandsworth
The successful candidate will be responsible for a geographical area, owning all the client relationships in that area, managing their clients care requirements by liaising with their GP’s, District Nurses, OT’s Discharge, pharmacist, family members etc, as well as their team of Care givers and supervisors, and will be required, at a minimum to template and maintain a 4-week call roster.
Duties/attributes:
Understand and build effective and efficient schedules around clients and carers. Ensure schedules are prepared, considering travel time, holidays, training and last-minute cancellations. Be responsive to changes in the schedule and liaise with relevant team members. Match carers to new clients in conjunction with client services team and arrange introductions. Ensure client schedules are matched to their needs, with the same carers and the same times each week, where possible. Develop excellent relationships with both clients and carers so both can enjoy positive experiences. Work with the recruitment team to ensure sufficient current and future staffing levels are met. Work to ensure new and existing care packages can be resourced and scheduled on a timely basis.
