Locum Band 6 Community Occupational Therapist

We have a fantastic opportunity for a locum Occupational Therapist to join a leading teaching hospital, based in Lancashire. They are seeking a qualified Occupational Therapist with previous Community Rehab experience. Job Description Can't have remote workers. The Community Rehabilitation and ESD Service are looking to appoint a dynamic, enthusiastic and forward thinking Occupational Therapist to join our service. The role will involve The role will involve assessing patients, planning treatment plans and coordinating support and equipment to meet the patient's needs; .working across both rehabilitation and ESD. Promoting continuity of care into the community setting and also working with the wider MDT to support long term rehabilitation pathways for patients in their own home's to support, increase or maintain their independence.
We are looking for a therapist with excellent communication skills who can demonstrate sound clinical assessment skills who has the ability to work autonomously as well as part of a team. The role will involve a holistic approach to encompassing physical rehabilitation and refer to wider community services. You will improve experience for patients and their families. We promote continuity of care for patients in the community setting. Supporting patients following falls and providing support for the Integrated Neighbourhood Team (INT).
